Add a new member or change member status
Additions
Recent additions to your family through marriage, birth or adoption are eligible for coverage through your plan. You may add them to your plan for coverage under Samaritan Choice Plans by contacting your local Human Resources Department. We encourage you to add your newborn through Human Resources as quickly as possible so you may begin seeking services for them immediately.
Changes
IMPORTANT REMINDER: All changes to your member information must go through your Human Resources Department. These changes are, but not limited to, new addresses or changes to eligible dependants.
