Out-of-area coverage

Samaritan Choice Plans provides local, statewide, national and international coverage to meet all your health care needs, either at home or away. Your primary servicing area includes Linn, Benton and Lincoln Counties.

When you travel within or outside the U.S.

All out-of-area, non-emergent and non-PCP services shall be considered non-preferred if provided through a non-contracted provider. Services provided by an out-of-area contracted provider will be paid at the preferred benefit level.

Urgent and Emergent services are always covered at the preferred benefit level, as are services provided by in-area contracted providers.

If you live outside the service area

If you or any members of your family live outside the Plan’s service area, you may choose one Primary Care Provider (PCP) in your hometown at the Tier 1 benefit level. A PCP visit is defined as services provided by a Pediatrics, Family Medicine, Internal Medicine, or OB-GYN provider. In those areas where Samaritan Choice Plans has contracted providers, out-of-area members must choose their PCP from our Tier 2 contracted provider list. Provider lists are furnished automatically and separately, at no additional charge. In order to receive the Tier 1 benefit for PCP services, Samaritan Choice Plans must be notified of your PCP selection.

If a change in residence occurs, contact Samaritan Choice Plans as soon as possible to discuss options for continuing coverage under this Plan.
